Medicare GP Chronic Condition Management Plan (GPCCMP)
For those that are eligible for the Medicare GPCCMP and have been referred by their doctor
Those with a chronic condition (e.g. diabetes) may be eligible for a GPCCMP, allowing up to five visits to allied health professionals per calendar year. Through the GPCCMP, Medicare will offer a benefit of $63.40 (as of 1 July 2026) which may either be mixed or bulk billed depending on concession status. See below for GPCCMP fees.
Please note that those attending without a valid Commonwealth Seniors Health Card or Pensioner Concession Card are not bulk billed. This means that we require an upfront payment of the below fees, and $63.40 will then be deposited back into your nominated bank account after we submit the online claim.
